How do I complete this program and obtain a document to send to H.R. showing I quit smoking on 11/11/20 ?

I am not sure how to answer this question, but going to put my administrators icon Mark down here, so he will come sometime today and answer you...Please know you can quit smoking.  Be sure to read at My EX Plan | BecomeAnEX and keep close to the support site for help...~ Colleen

Some members here are sponsored and will see information in the Sponsorship area of Edit Profile | BecomeAnEX . Their sponsor will be automatically notified of completion of the program. If you do not see your Sponsorship as Active and a referring Health Plan or Sponsor then your employer or health plan provider will not be notified as they do not participate in the EX Program and there isn't any document to provide to them for completion of our program. If you think you should be sponsored but forgot to list your Employer and Health plan provider at registration, please use the Contact Us form to let me know.
