How do I mark a document as helpful?

For a badge it says I'm to mark a document as helpful but there is no button for it.

Which document were you trying to mark helpful?

I know some people were having trouble completing this quest in the past. They were able to mark a document as helpful but it wasn't giving them history.  Please let me know which document and I'll take a look.

In documents there should be two buttons right above the attachments section.

I scrolled through slowly but still don't see it on any of the documents.

What device(phone,tablet,computer) and browser are you using currently?

Could you provide a screenshot of what you see on this document? The Transition from Smoker to Non-Smoker 

You should see near the top right under the words "You'll get there. This is eminantly doable."
