When I did an internet search for nicotine rehab, I found a number of resources. Mayo Clinic is probably the best known - but it is also mentioned in the literature that, since addiction is addiction in whatever its form, most drug and alcohol rehab centers are also equipped to deal with nicotine addiction.
I suggest you give it a try to see what's out there. There may be one close to you!
