I assume that is from the home page, from the "Recent Activity" Widget   It's hard to tell where that exactly came from since it is copied and pasted text(FYI it notified everyone you @mentioned) and not an image. But I'm assuming here...

Documents will show up in the Recent Activity Widget but unlike questions/discussions they will not reappear when someone comments and there is new activity on Status Updates or Documents. I'm not sure why they chose different logic for this.
